Initial Application

Dave's Education Fund Application is comprised of a series of documents that gives the Selection Committee a complete picture of your academic career, values and skills.

Please fill out the initial application form below to create a profile. Once we have verified your eligibility we will grant you portal access by email to begin uploading the remaining documentation. All documentation is required no later than March 31, 2023 to considered.

We require a recommendation from a manager or supervisor of your choice. Those recommendation forms are available at Customer Service for your Manager to complete and submit to the Committee directly.
